Shoulder impingement is the application of excessive pressure, or compression on the rotator cuff.

Injury and excessive repetitive use of the shoulder, along with age and genetics may cause what is called the shoulder impingement syndrome.

It makes range of movement very limited and painful, and if not treated properly, can cause future shoulder problems.

What Is Shoulder Impingement?

Shoulder impingement is also known as painful arc syndrome.

Shoulder impingement is also known as supraspinatus syndrome, swimmer’s shoulder, thrower’s shoulder, rotator cuff impingement, subacromial bursitis, and rotator cuff tendonitis.

The rotator cuff is a group of muscles which assist in shoulder rotation and strength.

When a shoulder injury occurs, the most commonly affected muscle of the rotator cuff is the supraspinatas.

The rotator cuff muscles surround the shoulder joint, and if an injury occurs, the muscles respond by swelling.

Because the shoulder muscles are surrounded by bone, swelling causes the pressure with the muscles to increase, resulting in compression.

When compression occurs, there is loss of blood flow in the small blood vessels. When the blood flow decreases, the muscle will begin to fray, causing a great deal of pain when reaching up, or behind the back.

It will make any range of movement very limited and very painful.

If the injury lasts for a long period of time without treatment, the muscle can actually tear into two, like a frayed rope, resulting in a rotator cuff tear, and/or rupture of the bicep muscle of the affected muscle.

What is Frozen Shoulder?

Many people don’t know what Frozen Shoulder, or Adhesive Capsulitis is until they are diagnosed with the condition.

Frozen Shoulder is also sometimes known as Frozen Shoulder Syndrome.

Frozen Shoulder symptoms

The shoulder joint becomes stiff, because bands of tissue or adhesions develop. There may be less lubricating fluid or synovial, in the joint.

Limited, or no range of motion is the best indicator of frozen shoulder syndrome.

Frozen shoulder also lasts much longer than a pulled muscle due to overexertion.  A correct diagnosis, physical therapy, and pain medication will help to relieve you of frozen shoulder.

Recovery

It may take as much as 18 months to fully recover, and in some cases, surgery may have to be discussed as a frozen shoulder treatment option.

Types of frozen shoulder

There are primary and secondary types of frozen shoulder.

Primary frozen shoulder

Primary frozen shoulder actually has no known cause.

Secondary frozen shoulder

Secondary frozen shoulder is due to trauma, or immobilization due to surgery, stroke, or other systemic diseases.

Frozen shoulder treatment

The focus of treatment with frozen shoulder or adhesive capsulitis is to continue working on increasing range of motion, but also limiting the pain of doing so.

Gentle exercise along with pain medication or corticosteroid injections may be the best combination, before seeking other alternatives involving surgery.

Alternative medicines may also be very helpful in loosening tissue, reducing inflammation, and relieving pain if you are seeking frozen shoulder treatment.

Adhesive capsulitis, or frozen shoulder treatment, involves preserving as much mobility in the shoulder joint as possible, while controlling the pain.  When you develop frozen shoulder, the joint capsule thickens, and becomes tight.

Frozen shoulder medications

Medications containing aspirin and ibuprofen may help to control the pain, and inflammation, while stronger pain relievers and anti-inflammatory drugs may need to be prescribed in the most painful stage of frozen shoulder.

Frozen shoulder physical therapy

Physical therapy will usually help, using stretching exercises, to maintain mobility, and to prevent muscle atrophy in the affected arm.

Physical therapy alone may not be beneficial without pain-relieving injections, if the pain is too severe.

Corticosteroids may be injected into the shoulder joints, relieving the pain by treating the inflammation of the tightened tissue causing the pain.

Both therapy and injections used together seem to complement each other, so that the shoulder joint can be exercised to increase range of motion, with limited pain.

Frozen shoulder surgery and alternative treatments

If the frozen shoulder pain and immobility issues last for over 18 months, your doctor may recommend surgery or alternative shoulder treatments.

Alternative medicine such as acupuncture, inserting fine needles at specific points of the body to relieve pain may also be used.

Transcutaneous electrical nerve stimulation (TENS) is another alternative which delivers tiny electrical currents, delivered through electrodes, to key nerves, which in turn may stimulate the release of endorphins, or pain-reducing molecules to block pain.

Capsular joint distension is another alternative treatment for frozen shoulder. Capsular joint distension involves injecting sterile water into the shoulder joint capsule.

This replaces some of the synovial fluid around the joint, and can help to stretch the tissue, making it easier to move the shoulder joint.

Shoulder manipulation is used to help loosen the tightened tissue,  using general anaesthetic.

The doctor then moves your shoulder joint in different directions to loosen the tissue surrounding the joint.  This alternative is used less often, due to the risk of bone fracture.

What is frozen shoulder surgery?

Frozen shoulder surgery may include removing scar tissue and adhesions arthroscopically, using a lighted, tubular instrument inserted through small incisions around the shoulder joint.

Arthroscopic surgery may be used with shoulder manipulation under anaesthesia, where the scar tissue and adhesions are stretched or torn in order to restore range of movement.

Because it is important in frozen shoulder treatment, to exercise as much as possible, a physical therapist can be very important.

The therapist may use ultrasound treatments, sending sound waves to relieve the inflammation surrounding the joint.

Gentle exercises may be the most effective way to loosen the tissue around the shoulder joint, and your doctor or physical therapist will provide exercises to do at home.

Liquid heat is often used to relax the shoulder, before doing the specific exercises, and the exercises should be stopped if the pain experienced is too intense.

What Causes Shoulder Tendonitis?

The shoulder injury known as shoulder tendonitis occurs when there is an inflammation, irritation and swelling around the rotator cuff tendons of the shoulder joint.

If you suspect that you have shoulder tendonitis you need to see a doctor to have your shoulder x-rayed.

Shoulder tendonitis is a condition that affects both men and women of all ages but is known to be more common to people over the age of 40. Shoulder tendonitis can be very painful even just from a simple movement of the shoulder.

Shoulder Tendonitis Symptoms

Some of the symptoms of tendonitis  include pain with doing activities like raising your arm, or experiencing pain while sleeping on the injured side, weakness of the muscles and cracking sounds when you try to move the arm.

 Shoulder Tendonitis Main Causes

There are many reasons why shoulder injuries are likely to occur, and the most common one is overuse or repetitive strain on the tendon. Tendonitis may also involve the elbow, hand and bicep.

Tendonitis is often felt by individuals who are either sports professionals like golfers, swimmers and baseball players or individuals like factory workers whose work involves repetitive movements of the arms.

Shoulder injuries may also be caused by putting the shoulder under too much strain. This type of shoulder injury is common with body builders and athletes especially at the beginning of the training. It’s very important to use correct training techniques while exercising because carelessness may cause a life time injury to the shoulder.

Shoulder Tendonitis and Age

The age factor plays a big role in causes of shoulder injuries and tendonitis, as you get older the shoulder loses its flexibility and elasticity while the tendons get harder.

Other Causes of Shoulder Tendonitis

Anatomy is also another rare cause of shoulder injuries. Some people are born with a hooked acromion that often causes them to get tendonitis. Sometimes the tendon does not have a smooth surface for its movement making it inflamed and easily irritated, this may be treated by surgery.

Thermal injury may also be a cause of tendonitis, it is often experienced by individuals who are mountain climbers, rock climbers and hikers.

Patients with underlying conditions that weaken shoulder muscles like rheumatoid arthritis, psoriasis and gout are more prone to shoulder injuries. A direct blow to the shoulder or falling on their arms may cause tendonitis.

Shoulder Tendonitis Diagnosis

Shoulder tendonitis is diagnosed by a physical examination and an x-ray. An x-ray is very useful in diagnosing shoulder injuries because it will show if a bone spur is compressing the tendon. An MRI may also be conducted to check if a rotator cuff is torn.

Shoulder Tendonitis Treatment

Most shoulder injuries do not require surgery. They can be treated with medication and physical therapy and controlled with rest and refraining from activities that caused the injury in the first place. Mostly tendonitis goes away over time but if the problem persists then other options may include surgery. Usually minimal invasive shoulder surgery is conducted by an orthopaedic surgeon and the recovery time is usual fast.

Shoulder Tendonitis Prevention

Left shoulder pain in women is more common these days and one reason is because many jobs women do today are computer based.

You may need to sit at the computer for hours on end each day. Poor sitting postures can lead to shoulder pain, but there could be more serious causes of left shoulder pain so you need to see a doctor in case your shoulder pain is a symptom of something more serious.

Left shoulder pain needs immediate attention because it is one of the symptoms of heart attack.

If the pain is cardiac in nature, the pain radiates to chest, arm, neck and jaw. If left shoulder pain is combined with other symptoms like nausea, vomiting and shortness of breath, you need immediate medical attention.

Left shoulder pain is not usually very serious in nature unless it is cardiac in nature but you do need to rule this out by getting proper medical attention. Left shoulder pain after a fall or accident should also be immediately seen by a doctor.

Other reasons for left shoulder pain in women

The other causes of shoulder pain in women are quite varied.  Shoulder pain can be due to arthritis, muscular cramps, damage to disc, gas problem or upper back pain radiating towards the shoulder.

Avoid lifting heavy objects till your pain is relieved. Apply ice packed in a cloth to your shoulder for five minutes.

Women who work in office and are sure that their left shoulder pain is office related, can get relief by exercising regularly. There are some easy shoulder exercises that can be done at home. You don’t need to go to gym or get the help of a physical fitness trainer to do these exercises if you follow my program.

Shoulder Warm-up exercises

The first and foremost thing that you must remember is you should warm up your shoulders before doing the exercises. If you start exercising without warming up, your painful shoulder may be aggravated.

One of the first exercises that I show you is how to bend and swing your arms like a pendulum. It is important that you move your hands freely without any stiffness. It looks similar to an elephant moving its trunk and I call it the pendulum exercise.

Shoulder stretching exercises

Once you have done warm up exercises like the pendulum exercise you can proceed to do shoulder stretching exercises.

Stretching exercises are very helpful in relieving shoulder pain. Pull both your hands behind your back. Lock your palms together. Now stretch your hands as much as possible behind your back. Continue for ten counts and then relax for sometime.

Repeat the same rotating your left shoulder clockwise and anticlockwise will relieve you from the pain. You can also swing your left arm front and back in full swing slowly. This will loosen your shoulder joints.

What Causes Shoulder Tendonitis?

Most people have never even heard of Shoulder Tendonitis until they are diagnosed with it.

What is Shoulder Tendonitis?

Shoulder tendonitis is the inflammation, irritation and swelling of the tendons of shoulder joint muscles Rotator Cuff and Bicep.

Shoulder Tendonitis is usually caused by the overuse of the shoulder joint. The shoulder joint itself is the most mobile joint in the body and is a ball and socket type.

Ball and Socket shoulder structure

This is a bit technical, but you need to know this if you suffer tendonitis and the “Ball and Socket” analogy generally helps patients understand what is happening with their shoulder. The head of the humerus bone is the “ball” and glenoid of the scapula is “socket”. The size of the glenoid is smaller compared to the head of humerus, hence the contact between these two bones is rather limited.

Because of size variation in ball and socket the bony articulation provides little stability to the shoulder joint and the bulk stability is provided by soft tissues of the joint such as ligaments and tendons. Therefore the effects of overuse of joint are mainly on soft tissues. The rotator cuff muscle provide dynamic stability to the joint which is comprised of four tendons namely supraspinatus, infraspinatus, teres major and subscapularis.

Overuse of joint leads to wear and tear of these tendons causing tendonitis. The tendon of bicep muscle is located in the front of shoulder joint and provides anterior stability. When rotator cuff tendons are overworked the strain will be more on bicep tendon to stabilize the joint. This extra strain on bicep tendon due to rotator cuff tendonitis causes inflammation of bicep tendon.

Shoulder tendonitis is usually caused by one or more of the following things:

  • – Frequent use of shoulder joint in throwing motion or frequent of use of the arm in an overhead  position as in tennis or racquet sports, swimming and baseball.
  • – Age factor- Usually occurs after the age of 40 years
  • – Falling on a stretched arm.
  • – Direct blow to the shoulder joint.
  • – Frequent heavy weight lifting especially weight lifters.
  • – Work nature such as overhead assembly work, butchering.
  • – Job where worker uses an overhead pressing machine
  • – Other diseases which weaken shoulder muscles such as rheumatoid arthritis, gout, arthritis, psoriasis.
  • – Rarely infection and unusual drug reaction.

Anybody can develop shoulder tendonitis but it is more common in adults over 40 years of age. As age advances the tendons become less elastic and less tolerant to the stress. Shoulder tendonitis is the most common sports injury due to repetitive minor impact on the shoulder joint and sudden serious injuries in the sports. Professional baseball players, swimmers, tennis players and golf players are especially susceptible to shoulder tendonitis. In any sportsperson who has shoulder tendonitis, the primary cause is improper technique, which causes overload on the tissues including tendons leading to tendonitis.

Since the main cause for the shoulder tendonitis is overuse of the joint, the best treatment is prevention. The person needs to avoid or modify the activity that causes the problem. Improper posture, poor technique in sports and work related issues need to be addressed. In general, and especially in sports, shoulder activity should be started slow and then built up gradually. Warm up and stretch shoulder joint before any activity or sports. Stop the activity if any unusual pain occurs in shoulder joint.
